Pet Hotel & Liability Waiver

Our goal here at Collar & Comb is to provide a safe & comfortable boarding environment, just like your home, for your pet while you are away. Towards that goal, the following rules have been implemented to ensure the health and safety of your pet and our Collar & Comb staff. 🐶

Reminders

  • Kindly send screenshot of your pets vaccination at least 3 days prior to check in.
  • Pets should be clean & healthy for boarding. An assessment will be done before check in. 
  • Pets should be free of ticks and/or fleas. 
  • To fully confirm your booking with us, we require a 50% deposit of the total price of your pets stay. Payment can be made thru BPI online transfer or Gcash. Credit card payment is also accepted.


Collar & Comb has the right to refuse service should the pet be unfit to be boarded in our facilities. 

Check In / Check Out Time

HOTEL

  • Check in: 9AM-1PM
  • Check out: 9AM - 12PM
  • LATE CHECK OUT: Check out beyond 12pm will incur day care charges + overnight stay charges.

DAYCARE

  • Check in/check out times will be from 9am-5pm.
  • Please note that FINAL Check-Out time is until 5PM Only

We DO NOT accommodate check out beyond 5pm. If you are unable to check out before our closing time, your pet will be kenneled and cared for for the night. This will be charged accordingly.

Vaccinations

For the safety of all pets, proof of current vaccinations is required at least 3 days before the time of boarding drop-off for all animals. This proof must be in writing from the licensed veterinarian who administered the vaccinations. Please provide Collar & Comb a photocopy of the vaccination records as well.

Collar & Comb reserve the right to turn away any pet more than 30 days past due for vaccines.


🐶 Canine vaccination required are:

  • DHLPPI (Canine Distemper, Hepatitis, Leptospira sp, Parvovirus and Parainfluenza virus)
  • Rabies
  • Bordetella


🐱 Feline vaccinations required are:

  • Rabies
  • FVR-CP (Feline Viral Rhinotracheitis, Calicivirus, Panleukopenia)


Health & Wellness

  1. Pet Owner(s) understands and agree that in admitting my pet/s, Collar & Comb has relied upon my representation that my pet/s is/are in good health and have not harmed or shown aggressive or threatening behavior towards any person or any other pet.
  2. Pet Owner(s) need to ensure that pet(s) is/are clean prior to check in or you can avail our bath & dry service upon checking in. 

Fleas & Tick Control

Pets should be up to date with their tick and flea prevention/medication.

Liabilities

Owners may leave personal items such as blankets, toys, bedding (as long as it fits in the kennel. But please understand that we will make every effort to keep track of these items while your pet(s) is boarding but we are not responsible for these items if damaged.


I understand that I, the pet owner, am solely responsible for any harm caused by my pet (s) while my pet (s) is/are attending hotel/daycare at or walking with Collar & Comb Staff.

Basic Things to Provide & Instructions

  • Bowls for eating and drinking
  • Leash & collar
  • Food for the pet
  • Other items and feeding instructions
  • Cat litter box and litter sand

Emergency Situations

While my pet(s) is/are in the care and custody of Collar & Comb, if I am unreachable in the event of an emergency, I hereby authorize Collar & Comb, its agents, and/or representatives to seek immediate veterinary care for my pet. I understand that all costs in connection with, veterinary, medical or other treatment shall be my responsibility.


In case of emergency situations, please note that some 24/7 vet clinics charge an emergency fee - in case your pet/s need to be brought beyond 6pm. This in addition to any tests that need to be done and their consultation fee. If the vet will recommend to confine your pet for monitoring, we will follow this recommendation.

I hereby release and agree to save and hold harmless, C&T Holistic Pet Services Corporation (doing business under Collar & Comb), it’s directors, officers, shareholders, employees, assistant, members and agents from any and all liability, claims, suits, actions, loss, injury or damage of any nature or kind, or for any liability, claims, suits, actions, loss, injury or damage which I or my pet(s) may sustain or which may be caused in any way by my pet(s). I specifically, without limitation, agree to fully indemnify Collar & Comb for any and all such liability, claims, suits, actions, losses, injury or damage.
