Pet Grooming & Liability Waiver

Here at Collar & Comb we care about the safety and wellbeing of all pets and we want to assure you that we will make every effort to make your pet's grooming visit as pleasant as possible🐾 🧡

I understand with all grooming procedures there is an element of risk involved and a risk assessment has been carried out to reduce any risk as far as possible.

I agree that the groomer/Collar & Comb will NOT be held liable nor responsible for irritation, abrasion, patchiness or hair loss due to any pre-existing skin conditions or as a result of the process of grooming, de-matting, thinning, stripping, clipping or any stressful effects that grooming may have on young, geriatric, timid, or temperamental pets.

I understand that, occasionally, grooming can aggravate a current, or expose a preexisting medical condition. Such medical conditions could arise during or after the grooming procedure. In the event of this situation, The pet owner/guardian shall not hold the groomer/Collar & Comb liable. 

I have read, acknowledge and agree to the Collar & Comb Terms and Conditions that were provided at this time. I will notify any Collar & Comb Staff of any concerns I have at the time and discuss any outcomes with him/her.

I agree that all the information provided is complete and accurate to the best of my knowledge.

Emergency Situations: While my pet(s) is/are in the care and custody of Collar & Comb, if I am unreachable in the event of an emergency, I hereby authorize Collar & Comb, its agents, and/or representatives to seek immediate veterinary care for my pet. I understand that all costs in connection with, veterinary, medical or other treatment shall be my responsibility.

I agree to release Collar & Comb, its groomers, employees, assistants, owners, shareholders, and affiliates from any and all liabilities, claims, suits, actions, losses, injury, damage, expenses, and costs (including veterinary and legal fees) resulting from any service provided; or unintentional injury to my pet while under their care or afterward.

I grant permission to Collar & Comb to use photos/videos being taken of my pet in any of Collar & Comb's publicity (Instagram, Facebook, Tiktok, etc)
